Your Guide to Completing Your MN Clinical Supervision Plan Online

Welcome! As you navigate your clinical hours, Join The Constellation is here to serve as your dedicated outside reflective and additional supervision provider. Because you are maintaining your primary employment elsewhere, this plan allows the Minnesota Board of Social Work (BOSW) to legally recognize our specialized clinical supervision hours toward your independent license.

Because we operate entirely virtually, we use the Board's secure digital portal to execute this document together. Follow these instructions to add us to your supervision network.

Before our first session: Please log into your MN Board of Social Work Online Services portal (our exact step-by-step matching guide is listed right below!).

• Fill out your section: Complete the form and list Nora Smyth, LICSW as your supervisor.

• Give me a heads-up: Once you submit your portion, drop me a quick email or text to let me know! The Board does not notify me automatically, so your message alerts me to log into my supervisor portal and digitally sign off to activate your plan.

💡 Forgot to do it before day one? Take a deep breath—we can fix it. The Minnesota Board of Social Work requires this plan to be filed within 60 days of starting supervision. However, to guarantee that 100% of your very first group hours legally count without any state processing delays, we want to knock this out as close to our start date as possible for total peace of mind!

DIRECTIONS

• Step 1: Go to the Website
  Open your internet browser and head to the Minnesota Board of Social Work Official Portal.

• Step 2: Log In
  Type in your regular username and password to access your licensee dashboard.

• Step 3: Find the Plan Form
  On your main account dashboard menu, click Submit a Supervision Plan. Note: Minnesota law requires to submit this online plan within 60 days of starting

• Step 4: Set Your Status & Employment Details

  • Plan Type: Select Revised Plan and check the box for Additional Supervisor. This protects your on-site supervisor's status.

  • Employment Section: Do not list Join The Constellation as your employer. Enter the exact legal name and address of the primary agency where you are currently employed and physically working.

• Step 5: Link Your Constellation Supervisor
  Select Add Supervisor in the portal interface and add my credentials to route the file to me:

• Step 6: Complete the Clinical Practice Addendum
  The online portal will prompt you to attach a Detailed Description of Clinical Social Work Practice. This outlines the scope of clinical work you perform at your primary job, which serves as the focus of our reflective supervision. Ensure your text addresses:

  • Client Population: The demographic range and clinical diagnoses you treat at your primary agency.

  • Clinical Modalities: The specific psychotherapeutic modalities (e.g., CBT, DBT, EMDR) you utilize.

  • Diagnostic Process: Your role in utilizing the DSM-5 and clinical screening tools to determine differential diagnoses.

• Step 7: Electronically Sign, Submit, and Notify Nora
  Review your data, read the final licensing attestations, and click Submit. Close the page, open your phone, and text or email Nora right away to say: "I just submitted my plan!"

• Step 8: Nora Signs Off
  Once you give me the heads-up, I will log into my matching supervisor portal, complete my required self-attestation hours, and hit approve. The system locks it in, and your hours are officially secure!
